Pricing

Adults: $100/month ($180 for couples) 

Children 0-18: $35/month (with an adult membership)

Family Max: $285/month (single family living in the same household)

There will be a one time $100 enrollment fee per person/household.

Frequently Asked Questions

How does the Revize365 membership work?
Once you are enrolled as a member, you will have direct access to your provider.  Your DPC membership will cover any day-to-day health care needs that can be managed via televisit, over the phone, text, and e-mail as appropriate. Same day and next day appointments available.
Your membership also includes one comprehensive annual exam. 
Revize365 members also receive discounts on specialty services such as medical weight loss, peptide Therapy, NAD+, and nutrient injections. 

Do you accept insurance?
While we don't bill insurance for your membership, you may still use your insurance to help cover any labs, imaging, specialty referrals, hospitalizations, etc. 

Can I contact my provider after hours and on the weekends?
While we ask that routine concerns and needs be addressed during normal business hours, you are certainly permitted to contact your provider with urgent concerns after hours. We provide all new members with a "how to" guide on utilizing our services for both routine and urgent needs.
